Koty Cook

Koty Allen Cook, 17, of Taylorville died at 5:14 p.m. Monday, June 29, 2009, in Taylorville Memorial Hospital.

He was born August 30, 1991, in Springfield, the son of Crystal Cook of Taylorville. He was a member of St. Mary's Catholic Church.

Surviving are his mother and step-father: Crystal and Robert Gordon of Taylorville; sister, Mickayla Cook of Taylorville; grandparents: Lee and Joyce Cook of Assumption and Kenneth and Tineal Fisher of Taylorville; great-grandparents: Robert and Shirley Fisher of Mt. Auburn and Frank and Glenda Kalka of Taylorville; uncle, Kent (wife Becky) Cook of Mt. Zion; cousin, Vincent Cook of Mt. Zion; and several great-aunts, great-uncles, and cousins.

Private family services will be held, with Father Dan Willenborg officiating. Burial will be in Oak Hill Cemetery.

Visitation will be held from 5:00 to 7:00 p.m. Wednesday, July 1, in Shafer-Perfetti Funeral Home.
